If client is UNDER 18 YEARS OF AGE, the legal guardian MUST:** - **Make Account with Client’s (Student’s*) Name 2. Fill out Intake Form PRIOR TO APPOINTMENT - located on number 2 after clicking button “I’m Done” 3. Sign Consent Forms PRIOR TO APPOINTMENT - same location as above
